Abstract

La prise en charge des cancers est aujourd’hui une priorité de santé publique. Pour cela se développent de plus en plus de nouveaux traitements dits de précision, et ce que l’on appelle la médecine personnalisée, à la maladie mais aussi au patient. Parallèlement depuis quelques années ce que l’on nomme « intelligence artificielle » connait un incroyable essor en santé, en même temps que les données disponibles se massifient et se complexifient. Parmi les méthodes issues de l’intelligence artificielles figurent les réseaux bayésiens, méthodes graphiques probabilistes utilisées dans de nombreux domaines et utiles à bien des égards pour résoudre des problématiques de classification et de prédiction. Leur fonctionnement est facilement compréhensible (évitant l’effet « boite noire »), ce qui en fait de très bons candidats pour l’élaboration d’outils d’aide à la décision. L’objectif de cette thèse est d’explorer l’utilisation de certaines méthodes issues de l’intelligence artificielle (les réseaux bayésiens) afin de modéliser des données médicales complexes pour optimiser la prise en charge thérapeutique de patients atteints de cancers. Elle se focalisera sur des médicaments à marge thérapeutique étroite utilisés en oncohématologie pédiatrique : la ciclosporine et le busulfan. La première partie du travail contextualise le problème et explique les grands concepts appliqués dans la seconde partie, qui présente le travail scientifique visant à faire la preuve de concept de l’utilisation des réseaux bayésiens dans les problématiques pharmaco-thérapeutiques complexes et à démontrer leur intérêt à travers d’exemples et d’outils d’aide à la décision développés. Today, cancer care is a public health priority. To this end, more and more new so-called precision treatments are being developed, and what is called personalized medicine, for the disease but also for the patient. At the same time, in recent years, what is called "artificial intelligence" has experienced an incredible boom in health, at the same time as the available data has become massive and more complex. Among the methods derived from artificial intelligence are Bayesian networks, probabilistic graphical methods used in many fields and useful in many ways to solve classification and prediction problems. Their operation is easily understood (avoiding the "black box" effect), which makes them very good candidates for the development of decision support tools. The objective of this thesis is to explore the use of certain methods derived from artificial intelligence (Bayesian networks) to model complex medical data to optimize therapeutic management of patients with cancer. It will focus on drugs with a narrow therapeutic margin used in pediatric oncohematology: ciclosporin and busulfan.
