This study conducted an online-based anonymous survey using a questionnaire. The research team used Google Forms. The link was disseminated in Facebook Group Page and Chats of the Occidental Mindoro State College and circulated the survey link to their network members. This study was approved by the Occidental Mindoro State College Research and Extension Council. The online survey has attached information on the research objectives as well as the significance and importance of the study. Participation in the study was voluntary and this was explained to the participants that they have the option to answer the questionnaire or not. Participant were informed of right to withdraw from research or limit participation if they become uncomfortable. Participant were also informed that the refusal to participate in the study or when there is a decision to withdraw from the study will not affect the health services they will be receiving. Also, the respondents were informed that there will be no further study-related activities after the survey. Likewise, the participants were advised that they can be provided the result of the study once they express the interest to know the results after the analysis and after completion of the final draft. The conduct of the survey complied with the policy stated in the Philippine Republic Act No 10173 or the Data Privacy Act of 2012. Complete anonymity of the research participants were observed. Email were collected but only for assurance that there will be no duplication of response. Specific information on the questionnaires could not be linked to specific individuals. Access to the data were limited to the researchers only. Unless required by law, the name of the participants will not be disclosed outside the research clinic. Names are available only to the following people or agencies: the principal investigator and staff; and authorized representatives of the principal investigator; ethics committees and health authority.
